Would anyone have a template with respect to formalizing the engagement of an “advisor” to a private company board of directors?  This is for a small startup company in the biomaterials field.  I imagine it would be some type of consulting agreement, but given that the candidate would be an advisor to the board of directors, I am wondering if there are any particular clauses that would need to be incorporated.

Any insight my GPC colleagues could provide me would be greatly appreciated.
